Yoga for Osteoporosis: What the Science Really Says About Bone Loss, Strength Training, and Where Yoga Therapy Fits In


If you're in your 40s or 50s, you've probably started hearing the word "osteoporosis" more often — maybe from your GP, maybe from a friend's DEXA scan results, maybe from a nagging worry about your own bones. It's a subject that touches almost every woman navigating perimenopause, and it deserves a clear-eyed, honest look rather than either panic or false reassurance.

As a yoga therapist in training, I want to share what the research actually shows — including where yoga helps, and where it isn't enough on its own.


The bone loss window: why timing matters


Bone is living tissue. Until about age 30, your body builds more bone than it breaks down. After that, the balance gradually tips. But there's one window where the tipping happens fast: the year before your final period through about two years after it. Research shows this short window can account for nearly 40% of all the bone loss a woman experiences between menopause and age 80.


Some women lose bone faster than others during this transition. Studies tracking bone density through perimenopause have found that roughly a quarter of women are "fast losers," dropping more than 7% of bone density in just two years, while others barely lose any. The decline is closely tied to the drop in ovulatory cycles and the resulting fall in oestrogen, which normally helps keep bone formation and bone breakdown in balance.

The practical takeaway: perimenopause — not just post-menopause — is the window to act, not the moment to wait and see.


What the science actually says about yoga and bone density


You may have come across headlines claiming yoga "reverses osteoporosis." They're referring to a well-known study by Dr Loren Fishman, a US physiatrics, who had a group of people with osteoporosis or osteopenia practise 12 specific yoga poses most days for around two years. The results were genuinely encouraging: bone density improved in the spine and femur for those who stuck with the practice consistently, with no reported injuries across the study.

But it's worth reading the fine print. The gains were modest, driven mainly by the small group who practised consistently over many months, and some researchers have questioned whether the changes were large enough to be reliably measured by DEXA scanning. The honest summary is: yoga appears to support bone health and is very unlikely to cause harm when practised well, but it isn't a stand-alone substitute for the loading that resistance training provides.


Why resistance and weight-bearing training matters so much


This is the part the wellness world sometimes underplays. Multiple systematic reviews now confirm that progressive resistance training — think squats, deadlift patterns, weighted carries, machines, or bodyweight strength work — produces measurably greater increases in bone mineral density than yoga or walking alone, particularly at the spine, hip, and femoral neck.


A few consistent findings across the research:


  • Load matters more than duration. Very light activity like gentle walking doesn't provide enough mechanical stress to stimulate meaningful bone remodelling.

  • Higher intensity tends to outperform lower intensity. Training at around 70–85% of your one-rep max has shown stronger bone benefits than lighter-load training in postmenopausal women.

  • Frequency matters too. Training three times a week generally outperforms twice a week for bone density gains.

  • Bone responds to novel, progressive stress. This is why the same 12 yoga poses, held statically, plateau in effectiveness — bone adapts and then stops responding unless the demand increases.


None of this means yoga is pointless. It means yoga and resistance training are solving different problems.


So where does yoga therapy actually fit in?

This is where your practice and your osteoporosis prevention plan can work together rather than compete:


  • Fall prevention. Most osteoporotic fractures start with a fall, not with bone density alone. Yoga's balance, proprioception, and coordination benefits directly reduce fall risk — arguably one of its most protective effects for bone health.

  • Safe spinal loading and alignment. A knowledgeable yoga therapist can teach weight-bearing standing poses and safe spinal extension work that complements strength training, while avoiding deep forward folds or aggressive twists that carry more risk for someone with low bone density.

  • Nervous system regulation. Chronically elevated cortisol actively breaks down bone. The stress-reducing, parasympathetic effects of a well-designed yoga practice — breath work, restorative poses, yoga nidra — address a genuine biological driver of bone loss that lifting weights alone doesn't touch.

  • Building the confidence to lift. For many women who feel disconnected from their bodies or intimidated by a weights room, yoga therapy can be the bridge — building body awareness, core stability, and confidence that makes starting resistance training feel possible rather than daunting.


The honest, whole-picture approach


If bone health is on your mind, the evidence points to a combined approach: progressive resistance training as the primary driver of bone density, weight-bearing movement for general skeletal loading, and yoga therapy as the layer that protects you from falls, supports safe movement patterns, and calms the stress response that quietly undermines bone health in the background.
